　 大腸

　大腸（結腸、直腸）および肛門に関する治療を行っています。大腸がんに関しては集学的治療（手術、抗がん剤、放射線治療、免疫療法）や最新のロボット支援下手術や腹腔鏡下手術などを駆使して安全で確実性が高く、患者さんの負担が少ない治療を提供しています。東京医科大学病院では臨床研究として2010年2月に日本で2番目にロボット支援（daVinci ダヴィンチ）での大腸手術を導入し、現在まで多くの患者さんに対して積極的に行ってきました。2018年にロボット支援直腸手術が保険適応となり、その後ロボット支援下手術の件数は飛躍的に増加しています。現在、東京医科大学病院では最新式の daVinci Xi 2台が稼働中であり体の負担が少ないロボット支援下手術と腹腔鏡下手術を合わせた低侵襲手術を80％以上の大腸がん患者さんに行っています。また、根治性が担保できる直腸がんに対しては括約筋間直腸切除術（ISR）や低位前方切除術などの肛門温存手術を積極的に行っています。

　大腸がんに対する抗がん剤治療も積極的に行っています。抗がん剤治療に関しては効果と副作用のバランスを考えながら、相談のうえで患者さんが納得できる治療を行います。また、潰瘍性大腸炎(Ulcerative Colitis: UC)とクローン病(Crohn's Disease: CD)を代表とする炎症性腸疾患（Inflammatory Bowel Disease: IBD)に対する治療も積極的に行っています。IBDは腸管の免疫異常によって発症する未だ原因不明の難治性腸疾患群を指します。その歴史は比較的浅く、1970年代の日本では非常に珍しい病気でした。しかし多くは若年期に発症して再燃と寛解を繰り返す慢性的な経過をたどるため、累積患者数は増加の一途をたどり、今では決して珍しい病気ではなくなっています。ほかに慢性便秘症、大腸憩室症などをはじめとする良性腸疾患についてもストレス社会、高齢化社会、そして食事、生活スタイルの欧米化などに伴い、患者さんの数は年々増加傾向にあります。良性腸疾患は悪性腸疾患と異なり内科治療が主ですが、中には患者さんの生活の質（Quality Of Life: QOL）を大きく損なうため手術を含めた外科治療が必要となることもあります。われわれはそのような現代そして未来の医療ニーズに応えるべく、炎症性腸疾患・良性腸疾患に対しても専門的な外科治療を提供します。

　炎症性腸疾患・良性腸疾患の多くは内科治療が優先的に行われます。内科治療で効果が不十分な場合やQOLが著しく障害された場合あるいは緊急手術を要する場合などに手術が必要となります。そのため内科、小児科や多くの医療スタッフからなる炎症性腸疾患・良性腸疾患センターを通じて適応を見極めながら必要に応じて外科治療を行います。また外科治療に関しては大腸外科グループ医師の協力のもと治療にあたっています。
